Carbamazepine: Out with the old, in with the new?
CBZ [Tegretol] is now widely accepted as a first line drug for focal epilepsy and has become the standard comparator for new drugs being licenced for focal epilepsy. The positive effect of CBZ on mood made the drug even more attractive. CBZ also has positive pregnancy data. Decades of usage have enabled us to predict when and how to safely use and manage CBZ.
Unsurprisingly, adverse effects are common in people taking CBZ. The key metabolite believed to be responsible for many adverse effects of CBZ including drowsiness, dizziness, and diplopia is the carbamazepine-epoxide, was identified in 1982.

Carbamazepine toxicity
Carbamazepine overdose has a predictable dose-dependent CNS depression and anticholinergic effects. Carbamazepine if unique in the anticonvulsants that is also has sodium channel blocking effects and can act similarly in overdose to a tricyclic antidepressant and the role of sodium bicarbonate should be remembered.
Carbamazepine-related antiepileptic drugs for the treatment of epilepsy - a comparative review
Older medications like Tegretol, Tegretol XR, and Carbatrol are inexpensive compared to the newer medications: Trileptal, Oxtellar XR, and Aptiom.
Cure Epilepsy
Carbamazepine (kar ba MAZ e peen) has been approved by the FDA to treat focal (partial) seizures with complex symptomatology (psychomotor, temporal lobe). Patients with these seizures appear to show greater improvement than those with other types.
Dibenzazepine Agents in Epilepsy: How Does Eslicarbazepine Acetate Differ?
Carbamazepine (CBZ), oxcarbazepine (OXC), and eslicarbazepine acetate (ESL) belong to the dibenzazepine family of antiepileptic drugs and are all thought to primarily act as sodium channel blockers (SCBs). However, ESL is structurally distinct from CBZ and OXC, resulting in differences in metabolism, pharmacokinetics, and pharmacodynamics.
What place do carbamazepine-related antiepileptic drugs have in the modern day treatment of epilepsy?
Carbamazepine (CBZ), oxcarbazepine (OXC), and eslicarbazepine acetate (ESL) are all members of the dibenzazepine carboxamide family. CBZ is the oldest in this class of antiseizure medications (ASMs) and one of the most used drugs to treat epilepsy worldwide.
Carbamazepine: Out with the old, in with the new?
CBZ was a big step forward in 1953, where epilepsy options were few, the world population was smaller and obesity was uncommon.

Tegretol
Carbamazepine is used to manage and treat epilepsy, trigeminal neuralgia, and acute manic and mixed episodes in bipolar I disorder. Indications for epilepsy are specifically for partial seizures with complex symptomatology (psychomotor, temporal lobe), generalized tonic seizures (grand mal), and mixed seizure patterns.
